Caring for the elderly in our communities<\/h2>[vc_column_text css=””]Here at TyneHealth we are privileged that our frailty nurse practitioners lead in the provision of enhanced healthcare services for older people living in care homes. Working in partnership with our colleagues in the care homes, local primary care networks and wider community services this service seeks to embrace a proactive approach to care delivery using the robust evidence base of comprehensive assessment and care planning.[\/vc_column_text][vc_row_inner][vc_column_inner width=”1\/2″]

How the service works<\/h2>[vc_column_text css=””]<\/p>\n

Older people can have the most complex of needs and addressing these needs can be hugely challenging.\u00a0 Whilst there is a strong evidence base demonstrating that older people benefit most when they have access to multidisciplinary teams working within integrated services, it is difficult to find anything happening inside older people\u2019s healthcare that has not been influenced by nurses.<\/p>\n

Currently the Care Home Service operates in the North West area of North Tyneside as well as in the Whitley Bay area. If you or your family are registered with the following link GP practices in each area, then our team of nurse practitioners will be working alongside you, your family and care givers, with your GP, to understand your needs and facilitate excellent care delivery.<\/p>\n
