Core20PLUS5 is a national NHS England approach to inform action to reduce healthcare inequalities at both national and system level. The approach defines a target population – the ‘Core20PLUS’ – and identifies ‘5’ focus clinical areas requiring accelerated improvement.
In North Tyneside there is a specific cohort of people the Core20PLUS5 programme focuses on and the aim is to understand this group and work with them alongside other services such as Adult Social Care, housing, mental health and addiction services. The cohort focused on are people who have a substance misuse flag and at least one mental health condition on their record. In North Tyneside there are over 7,000 patients registered with those on their record many of whom have attended A&E in the last 12 months.
Working with North Tyneside Local Authority our Core20PLUS5 team are working with individuals who have no connection or relationship with Primary Care with the aim of transitioning those individuals back into the GP Practice, developing trust with the individual in the process as they transition across. By visiting people in their homes and holding specific clinics (such as our wound care clinic in NTRP) our team hope to make a difference to people’s healthcare.
